Insurance/cash
AMPT accepts nearly all PPO insurance plans and select HMOs, Workers Compensation, most Medi-Cal insurance plans, and Medicare. To verify coverage for your specific insurance carrier please call our office or fill out a contact us request.
Our cash pay rates are $150 for initial evaluations and between $65-95 per follow up visit dependent on treatment.
Outsourced
Our clinic is best suited for clients who can move about independently. We currently do not have any pelvic floor specialists on staff.
If your client needs very basic help with pelvic floor we are happy to help, however if a specialist is necessary, they should be referred to a specialist.
